A United States Navy aircraft with two crew members on board has crashed near Mount Rainier in Washington state, triggering an ongoing search and rescue mission. An inquest jury has found that “multiple failings” at a UK immigration detention centre contributed to the tragic death of Colombian national Frank Ospina, who took his own life in 2023. The jury’s findings indicate that missed opportunities and inadequate care at the Harmondsworth Immigration Removal Centre worsened Ospina’s mental health crisis, ultimately leading to his death.
